Since the publication of a series of posters at ICAAC 2014, we finally know what\u2019s hidden behind the \u2018Carbavance\u2019 name.\u00a0 \u2018Carbavance\u2019 stands for the combination of meropenem with the B-lactamase inhibitor RPX7009 which The Medicines Company (TMC) inlicensed from Rempex Pharmaceuticals a few years ago.\u00a0 This is the \u2018Carbavance\u2019 which TMC will take into Phase 3 development.<\/p>\n

Until recently, \u2018Carbavance\u2019 was known as the combination of biapenem and RPX7009.\u00a0 Then, there was a new moniker, \u2018RPX2014\/RPX7009\u2019, also referred to as \u2018Carbavance\u2019 [1].\u00a0 All very confusing, maybe by design.<\/p>\n

Above\u00a0an excerpt from the most recent Investor Conference presentation\u00a0(Sept 23, 2014 which -somewhat \u00a0schizophrenically – gives us 2 \u2018Carbavance\u2019 definitions.\u00a0 Sometimes labeled \u201cmeropenem plus RPX7009\u201d, and at other times as \u201ca marketed carbapenem PLUS RPX7009\u201d, the decision to use meropenem was probably so recent that the slide makers had trouble catching up with the new realities.<\/p>\n

Okay, we get it: \u00a0As the beta-lactam work horse drug, biapenem was an penem with a clinical study program conducted in Japan but not in the US.\u00a0 As such, it would have taken some\u00a0time and effort to get the combination approved in the US.<\/p>\n

Hence, TMC decided to replace it with meropenem which already has a complete dossier with FDA.\u00a0 Presumably,TMC has permission from AstraZeneca to cross-reference the meropenem data.\u00a0 This way, only a small Phase 3 program would be needed, an important competitive advantage.<\/p>\n

The switch looks like a clever move from which\u00a0both companies stand to benefit.\u00a0 Except that biapenem now needs to find a new partner. \u00a0It could\u00a0be combined with the new beta-lactamase inhibitor\u00a0OP0595 just presented at ICAAC.\u00a0 Hopefully this new combination will not be called \u2018Carbavance\u2019 as well.<\/p>\n

It is high time to get rid of the confusing\u00a0\u2018Carbavance\u2019 name: This rose – by any other name – would smell a lot\u00a0sweeter\u2026<\/p>\n
